POLICE RECORDS CHECK FORM

Little League International and Little League Canada mandate that all adult volunteers annually file a Volunteer Application and a Police Records Check.

Please print and fill out one of each of these and forward to Pinecrest Little League's Safety Officer or the President.

There is no charge and all information is kept confidential. You will be contacted by either the Safety Officer or the President only if there is clarification or feedback required.
